Clinical Development Concept Completeness Check: Is the Study Concept Ready to Become a Protocol?
A concept sheet can contain many protocol details and still be strategically incomplete. Before drafting accelerates, the team should be able to explain what decision the study is intended to unlock, which evidence threshold matters and how the design connects to the next regulatory, payer, operational and financing gate.
When this diagnostic is useful
Use it before a protocol lock, pivotal-design decision, regulatory interaction, major vendor commitment, financing gate, partnering discussion or repeat-study decision.
Use it when different functions are giving individually reasonable answers that do not yet add up to one defensible development decision.
What to pressure-test
- Development decision and value hypothesis
- Clinical rationale and target population
- Objective, endpoint, estimand and comparator alignment
- Eligibility criteria tested against real patient availability
- Visit schedule, procedures and patient/site burden
- Safety monitoring, discontinuation and rescue logic
- Statistical assumptions and decision criteria
- Clinical development strategy and next study logic
- Regulatory interaction and submission purpose
- Country, site and execution architecture
- Evidence that would trigger redesign, pause or termination
Red flags
- The team can describe the protocol but not the decision it is meant to support.
- Feasibility begins after the population and schedule are already fixed.
- Regulatory purpose and payer relevance are implicit rather than written.
- There is no stated evidence threshold for redesign or stop.
Why this matters to capital
The concept stage is where design changes are still cheap. Once protocol writing, vendor contracting and site activation begin, missing decisions become change orders, amendments and lost runway.
What changes after a useful review
A useful diagnostic does not end with a score. It identifies the dominant uncertainty, distinguishes what is known from what is assumed, shows which decision is currently exposed and defines the smallest next action capable of changing that decision.
