The pivotal architecture is not only a statistical choice. It determines what can be claimed, how regulators and payers will challenge the evidence, what follow-on evidence may be required and how much capital remains exposed if the assumptions fail.

The decision problem. Teams are naturally attracted to designs that look faster or smaller. The downstream cost may be hidden: narrower labels, confirmatory obligations, payer uncertainty, weaker partnering credibility or a second study that was never included in the original financing plan.

A pivotal trial is one of the largest capital-allocation decisions in a biotech programme. The cheapest design is not necessarily the design with the lowest expected cost to value inflection.

What changes after a useful review

A useful diagnostic does not end with a score. It identifies the dominant uncertainty, distinguishes what is known from what is assumed, shows which decision is currently exposed and defines the smallest next action capable of changing that decision.
